How to use our online self service fill-in PDF forms
General Information
You will be able to fill out a form on the screen and print the form, or print the form and fill it out by hand. Once the form
—your signature is required. If applicable, you must obtain appropriate approval signatures and attach the processing fee before submitting it to the Registrar's Office.
Having difficulty opening up our undergraduate forms?
- try deleting cookies and clearing your cache
- try moving the slider bar located at the right to the top
- try downloading the latest version of Adobe Acrobat (free) and then open up the forms again
- make sure you have Adobe Acrobat version 7 or later (our forms use version 9)
- only have one version of Adobe on your computer (multiple versions cause problems)
- allow pop-ups from all University of Waterloo websites
- try closing your browser and try again
- try shutting down your computer and try again
- your place of employment may have a firewall to prevent you from opening up our forms; try from home
Instructions for using fill-in forms
- Once the form is open, click your cursor in the first
field to make it active.
- Press the tab key to move through the fields.
- For check boxes, position your cursor over the box.
When the shape changes to a pointer, you can click to make your selection.
- Use the reset button located towards the end of the form
to clear all fields on the form.
- To go back one field, hold the shift key and click tab or put the
cursor in the desired location.
- To add accents in the name fields on the Intention to Graduate
and the Request for Duplication/Replacement Diploma forms:
open up a new Word document, Insert Symbols, make sure the Font is equal
to (normal text), select the letter with symbol, click insert, and then
click close. Now you need to copy the letter with symbol and paste into
the correct position in the name field.
- When you have completed your form, click on the Print button (located
beside the floppy disk icon) on the Adobe tool bar and select the desired
number of copies.
How to submit the completed form
Once you have filled out the form, send it to the Registrar's Office using regular mail or fax the form to us. Do not send the completed form by email attachment (form will come through as a blank form).
