The Registrar's Office requires a cover page for all final examination masters printed through the Registrar's Office. The cover page is the first page of the exam master. The cover page is printed on white paper. The instructor must specify on the cover page whether the exam is closed book, open book, or special materials. If designated as special materials then the instructor must specify on the cover page what materials are allowed. All exam papers will be printed double-sided unless the second side of the sheet is designated for rough calculations as indicated on the cover page.
There are three different ways to create the exam cover page: word document, filling out an Adobe PDF form, or creating your own.
Save the Exam Cover Page word document. If you need to change the marking scheme section it can be deleted or modified.
You can fill out the exam cover page (pdf) online and print it or print it (to insert in a typewriter). If you require more than one cover sheet, don't save the pdf, close the pdf, and open it again for each additional cover page. A reset button is normally added to this type of page; however, to avoid printing the button on the page, it has been excluded. When you mouse over the field, it will give you an example of how the field should be filled out.
You may create your own exam cover page as long as all of the information listed in the top two sections of the sample exam cover page (pdf) is included in your cover page.
Adobe Acrobat Reader is required to display and print the exam cover page and the sample exam cover page pdfs. You can download the latest version of Adobe Acrobat free of charge.
